Membranes cannot be shipped out on Friday’s for weekend delivery to due quality control reasons.  There must be someone at your office the following day after an order is placed to receive shipment.  Orders received and paid for before 2pm EST will ship out same day, order placed after 2pm EST ship out the following day.

Using amniotic membranes for the first time can be challenging.  As part of being a good business partner we offer on your first membrane a no questions asked guarantee.  Should your membrane fall, fold, or otherwise be unusable in your first attempt, just let us know and we will send you out your replacement.  First time/initial orders only.

Billing, Payment Terms, and Late Fees:

CR Surgical Supply (“Company”) requires payment at the time of purchase.

All invoices are in U.S. dollars, and prices are subject to change without notice. The Company ships only within the United States. Payment terms must be met as agreed, and any unpaid invoices will accrue interest at 18% per annum (equal to 1.5% per month), or the maximum rate allowed by law, whichever is lower. A $25.00 monthly fee applies to all unpaid accounts.

Payments can be made by check or credit card (Mastercard, Visa, American Express, and Discover). If payment is not received within the stated terms, the Company may withhold future shipments. Acceptance of partial payment does not waive any rights. Invoice amounts are considered correct unless disputed within 10 days of receipt. Taxes:

Unless the customer provides Company with a valid and correct tax exemption certificate applicable to the product ship-to location before Company’s acceptance of the order, the customer is responsible for all sales, use, and other taxes associated with the order. Returns Policy

Due to the nature of human allograft tissue, CR Surgical Supply does not accept returns of any allograft products under any circumstances.  This includes claims related to product quality or suitability. This policy ensures the integrity and safety of our products and maintains compliance with regulatory standards. 

Defective Product Reporting

In the rare instance that a product is believed to be out of specification (OOS) or presents any issue that compromises its intended use, we ask that you contact CR Surgical Supply by phone 404-480-8998 or email: info@crsurgicalsupply.com within 24 hours of receipt or first identification of OOS.  Adverse events must be reported directly to CR Surgical Supply within 24 hours of identification. 

Out of Specification (OOS) Reporting: If the product is perceived to be defective or not as described in terms of appearance, labeling, or handling instructions, the customer must notify CR Surgical Supply within 24 hours of receipt or at the first . Reports should include detailed descriptions and any relevant photographs or evidence of the defect.

CR Surgical Supply will review all claims and may request additional information as part of the investigation.

Please note, due to the nature of allograft tissue, replacement or reimbursement options are not available. However, CR Surgical Supply will work with customers to address and resolve any concerns related to product quality.

Limitations of Liability

CR Surgical Supply disclaims all liability for improper handling, storage, or use of the product that does not adhere to the provided instructions. Customers are responsible for ensuring that products are used within their intended clinical applications and that they have met all requirements for product handling.
